Always use ATMs inside banks during the day. The outdoor ones are risky. Mine got stuck once in Loja and no one could help till the next business day.

Decline is the better choice..... Accept is just the ATM taking a cut off your withdrawal with a bad exchange rate. Been there, lesson learned.
